Know Your Medicine Programme
The Know YourMedicine Programme provides you with a unique opportunity to have an uninterrupted, one-on-one session with the Pharmacist Educator to discuss all your prescription medicines and over-the counter supplements you take. The session usually lasts for an hour during which time the Pharmacist goes over side effects, drug and food interactions, best use and other special instructions related to your medicines and supplements. Medical conditions are also discussed, and you will have a chance to ask questions, should you have any. The mechanism of action of each medication is explained, particularly in relation to how they work to treat each condition. At the end of the first session, you will be provided with a detailed summary of your prescriptions and supplements. This will help you understand why you’ve been given each medication and hopefully improve compliance and ultimately health outcomes. If your medications need adjusting this will be done in conjunction with your usual physician. Your blood pressure and blood glucose will usually be checked during each review. Further follow-up sessions will be recommended if needed.
Sessions
Sessions are available Monday to Thursday from 9:00am to 4:30pm, and on Friday from 9:00am to 11:00am, subject to availability.
Before Your Appointment
To help you get the most out of your first appointment, you may be asked to complete a New Patient Information Form, where you should list your current medications and supplements. This allows the Pharmacist Educator to prepare a summary sheet in advance. Please bring all of your medicines (including over-the-counter items such as Advil or Ibuprofen) or a complete list with the names and dosages.
In addition, please bring along your most recent blood test results (you may need to request these from your doctor’s office). This will help the Pharmacist Educator to assess the efficacy, as well as the safety of your current medications.
If you use a Continuous Glucose Monitor (CGM), such as FreeStyle Libre or Dexcom G6/7, or a standard glucose meter for finger-prick testing, please bring these devices with you as well.
