This privacy notice explains what personal information the Bermuda Diabetes Association (“BDA”) collects, how it is used and the purpose for which it is used.  The notice covers every individual’s right to review information pertaining to them and/or to request it be corrected or destroyed. BDA only collects personal information which is adequate, relevant, and not excessive for the purpose collected and is kept no longer than necessary for such purpose.  BDA endeavours to ensure that personal information is accurate and kept up to date.

We use the following information to provide health services and medications, including delivery:

о  Address, email address(es) and phone number(s)
о  Date of birth
о  Gender
о  Biometric information, e.g., blood pressure, glucose level, HbA1C, etc.
о  Prescriptions and medical history
о  Health information (including diabetes diagnosis, dietaryrequirements, allergies, medical referrals, and health conditions)
о   Health insurance information
о   Emergency contact information
о   Record of consent, where applicable
о   Identification documents, income and other financial information –only in instances which involve a request for financial assistance

We use the following information for service updates, diabetes and health awareness, marketing or communication purposes:
о   Name
о   Address, email address(es) and phone number(s)
о   Gender
о   Limited health information pertinent to tailoring communications tomeet your interests and needs (e.g., diabetes diagnosis, family history ofchronic disease, and type of health insurance coverage)
о   Records of consent, where applicable
о   Marketing/subscription preferences
о   Recorded images, such as photos or videos
о   Membership and donation records

We use the following information for researchor advocacy purposes:
о   Full name
о   Address, email address(es) and phone number(s)
о   Date of birth
о   Gender
о   Biometric information, e.g., blood pressure, glucose level, HbA1C,etc.
о   Health information (including diabetes diagnosis, dietaryrequirements, allergies, medical referrals, and health conditions)
о   Family health information
о   Racial or ethnic origin
о   Health insurance information
о   Records of consent, where applicable
о   Recorded images, such as photos or videos

We use the following information for recruitment, employment, and the participation of volunteers:
о   Full name
о   Address, email address(es) and phone number(s)
о   Date of birth
о   Gender
о   Police check
о   Copies of passports or other photo ID
о   Employment history (i.e. job application, employment references)
о   Education and credentials
о   Performance
о   Leave

Third-parties IT providers
BDA processes data, such as client records, internally by using trusted third-party software which complies with BDA’s privacy and confidentiality standards.

We share information through the use of third-party IT providers as follows:

о   To respond to, provide, or communicate with your other healthcare providers, only with your consent.
о   Upon request, to transfer prescriptions to or from pharmacies.

Website

The BDA website only collects personalinformation if you voluntarily provide it. Should you choose to sign up for events, volunteer opportunities, make a donation, apply for membership, orother activities, the information you submit will be used for that specific purpose.
